Shorefront Y NNORC Health Consultation Program
Brighton Beach Library has partnered up with The Shorefront Y’s NNORC Health program providing health focused outreach to seniors (60+) to help them better manage their health and successfully stay in their homes and community.
The NNORC health professionals from Partners in Care, VNS Health, will be in the Brighton Beach Library once a week.
Join us!
Services (provided free of charge):
- blood pressure screening
- consultations on how to manage chronic conditions and medication management.
- motivational interviewing to help seniors follow healthy diet and exercise regiments
- connecting seniors to services and programs in the community.
