More than 37 million adults have diabetes according to the Centers for Disease Control and Prevention (CDC). Join us to learn about the differences between Prediabetes, Type 1 and Type 2 diabetes, how to lower the risk of Type 2 diabetes and it's complications, and ways to manage Type 2 diabetes once diagnosed.

Presenters: Dr. Gwendolyne Anyanate Jack, MD, MPH, and Dr. Jenny DeJesus, DNP, APN-BC,ADM-BC, CDCES, from NewYork-Presbyterian/Weill Cornell Medicine.

Dr. (Anyanate) Gwendolyne Jack is an Assistant Professor of Clinical Medicine at Weill Cornell Medicine’s Division of Endocrinology, Diabetes, and Metabolism. She received her Bachelor of Arts degree from New York University. She earned her Masters of Public Health from Columbia University Mailman School of Public Health. She earned her Medical Doctorate at Sidney Kimmel Medical College in Philadelphia. She is board certified in Internal Medicine and Endocrinology. She focuses on management of pre-diabetes, Type 1 and type 2 diabetes mellitus. She is the Lead of the Ambulatory Diabetes Program at Weill Cornell Endocrinology Faculty Practice. She currently serves as the Social Determinants of Health(SDoH) Champion for Weill Cornell Medicine and is Directory of the Faculty Scholars in Health Equity program. She has received awards for Excellence in Medical Education and was named in the National Minority Quality Forum 40 under 40 Leaders in Health list. She has been an invited speaker to several academic and community presentations on prevention/management of pre-diabetes and type 2 diabetes mellitus, as well as served as coauthor on several publications. Dr. Jack is passionate about medical education, quality improvement initiatives, health equity, and enhancing the patient-physician-community partnership.

Dr. Jenny Dejesus, DNP, AGNP-BC, BC-ADM, CDCES is a Nurse Practitioners at Weill Cornell Medicine Division of Endocrinology. She obtained her BSN & MSN from Hunter Bellevue School of Nursing in NYC. Jenny is a board-certified Adult/ Geriatric Nurse Practitioner. She has been a Certified Diabetes Educator since 2007. Jenny is a member of the American Diabetes Association and the American Diabetes Educators Association
